Nurse Practitioner

We are currently seeking an experienced Hospice & Palliative Care Nurse Practitioner for a Per Diem position covering Onslow and surrounding counties.

• Implements palliative care program requirements
• Completes medical visits with patients per guidelines and coordinates care with the Palliative Care team as clinically indicated
• Conducts face-to-face encounters to provide re-certification information to the Medical Director/Associate Medical Director for hospice patients
• Provides clinical findings to the primary care physician, and/or supervising Medical Director/Associate Medical Director
• Communicates with patients/families at all levels
• Communicates with all agency personnel

• Master’s degree in Nursing required, with current licensure in the state of practice
• Certification as an Adult or Family Nurse Practitioner
• Experience in hospice or palliative care nursing preferred
• Knowledge of the framework, function, and organization of a hospice care program
• Excellent communication and organization skills
