Min Max Overview
Facilitates the delivery and evaluation of evidence-based comprehensive care to patients. Collaboration and referral to other healthcare providers, patient advocacy, teaching, and family counseling are major components of the role. Under the direction of a physician or designee, provides coordination within their department or work area.
QualificationsRequirements:
- Certification as a Nurse Practitioner required or FNP as determined by the department.
- Must possess and maintain licensing as required by Illinois State law for APN.
- Minimum of 1 year post-graduate school experience as an APN preferred.
- Valid CPR required. ACLS certification where required.
- Maintains DEA license where required.
- In collaboration with supervising physician(s), practices within professional scope and current standards to assess, diagnose, and treat patients.
- Follows protocols according to supervising physician’s direction, including encouraging the patient to follow up with their primary care physician. Partners with the physician(s) to promote effective patient care. Provides visit documentation and associated testing for review and signature. Communicates effectively with physicians when clarification is needed, especially when deviating from protocol or before ordering costly tests, treatments, or referrals.
- Completes thorough and timely documentation according to standards set by Medicare, Medicaid, and other payers, as well as MHS policies on medical records.
- Coordinates resources required by the patient.
- Collaborates with other healthcare professionals, patient advocates, teachers, and family counselors to formulate and initiate a healthcare plan. Routinely evaluates and modifies the plan to improve outcomes.
- Develops, implements, and evaluates plans to adhere to evidence-based quality indicators.
- Serves as a clinical consultant to other healthcare professionals.
- Reviews and utilizes current literature and research findings relevant to patient and family healthcare.
- Assists with implementing research findings and monitoring their impact on patient outcomes.
- Uses available data to analyze performance and develop improvement plans.
- Works to enhance advanced clinical expertise in their specialty area.
- Develops, implements, and evaluates educational activities specific to their specialty.
- Participates actively in community and professional programs.
- Supports financial goals related to revenue and expenses. Maintains productivity within budget. Communicates when volumes are low or opportunities for efficiency exist.
- Strives to meet customer service goals by providing high patient satisfaction. Reviews survey results and seeks improvements.
- Supports team goals and objectives.
- Performs other related work as required or requested.
