Job Summary
Baptist Health Registered Nurse, Case Manager — In-Patient Case Management Department, Baptist Medical Center South campus — full-time, day shift — is seeking a compassionate and detail-oriented individual to join our team. As a Case Manager, you will play a critical role in coordinating care for hospitalized patients, ensuring smooth transitions through the continuum of care, and working closely with the multidisciplinary team to optimize patient outcomes.
Key Responsibilities- Perform high-risk screenings and comprehensive assessments
- Coordinate care plans and post-acute services with accurate documentation
- Facilitate communication among providers, patients, and families
- Support safe and timely discharges through effective resource utilization

- Education Required:
Associate s Degree - Nursing
- Bachelor s Degree - Nursing
- 1 year Case Management Experience
