Home Health Nurse

Registered Nurse – Home Health / Hospice (8-Hour Shifts)

The Registered Nurse provides skilled nursing care to patients of all ages in their homes, coordinating care with physicians, interdisciplinary teams, patients, and families. This role is responsible for assessing patient needs, developing and implementing individualized Plans of Care (POC), ensuring high-quality, compliant documentation, and promoting patient education and self-care.

• Perform comprehensive patient assessments in stable and emergent home settings
• Develop, implement, evaluate, and document Plans of Care (POC)
• Provide skilled nursing care including wound care, IV therapy, pain management, catheter care, and medication administration
• Educate patients and families on disease management, medications, safety, and self-care
• Supervise and provide clinical direction to HHAs/STNAs/LVNs
• Coordinate referrals and ensure compliance with payer and regulatory requirements
• Participate in on-call rotation and interdisciplinary team meetings
• Maintain accurate, timely documentation using Homecare Homebase and OASIS

• Active state RN license
• 1–2 years RN clinical experience (required)
• 2+ years home health experience (required)
• Homecare Homebase & OASIS experience (required)
• CPR certification, valid driver’s license, reliable transportation & insurance
• Ability to work independently, travel locally, and maintain confidentiality

• Associate Degree in Nursing or Nursing Diploma (required)
