Position: Admissions Nurse RN (Registered Nurse)

RN ADMISSIONS AND DISCHARGES DUTIES:
  • Receives and gives report to other staff nurses and supervisor;
    Counts controlled medications with outgoing and incoming nurses according to procedure (in the absence of a second nurse).
  • Assigns and directs direct care staff and Licensed Practical Nurses when appropriate in resident care and nursing activities. Instructs and assists the nursing staff and auxiliary nursing personnel on nursing problems, unit management and related activities.
  • Carries out resident/patient care plans with consultation with the Unit Manager as necessary.
  • Assures safety of the residents at all times.
  • Assesses and directly observes resident in person to ascertain nursing care needs and prepares and carries out required care plans. Completes required documentation utilizing computerized medical record system.
  • Utilizes and ensures accurate and descriptive records of the treatment and care of residents utilizing computerized medical record system.
  • Assists with resident care as the position demands.
  • Makes rounds to all residents/patients at least twice a shift and PRN. Assesses resident/patient care needs and prepares and carries out nursing care plan.
  • Investigates employee problems/incidents; deals with them according to procedure in consultation with the Unit Manager and/or Supervisor when appropriate.
  • Notifies the Unit Manager and/or Supervisor of resident/patient problems (medical, physical, psychological, family, and physician), when appropriate.
  • Makes rounds with the physician, calls physicians for changes in resident condition (after notifying the Unit Manager and/or Supervisor when appropriate and transcribes physician orders. Insures prompt and accurate execution of orders.
  • Handles resident incidents, emergencies, transfers, admissions and deaths, according to procedure and with the assistance of the supervisor.
  • Participates in the assessment and care planning process.
  • Completes specific duties as required by shift assignment (i.e. night nurse checks each medical record for any outstanding orders).
  • Meets with direct care staff at start and end of shift to facilitate report on resident status.
  • Completes evaluations on direct care staff in consultation with the Unit Manager and/or Supervisor.
  • Reports all cases of alleged violations involving mistreatment, neglect or abuse, including injuries of unknown sources.
  • Communicate with all members of the interdisciplinary team for upcoming discharges and potential admissions.
  • Ensure a seamless transition for all admissions coming into the facility by obtaining proper paperwork, following the admission guideline, coordinating with on call physician for medication and delegating and communicating to nursing staff.
  • Be versed in wound care and treatment to assist with nursing staff throughout the day.
  • Prepare discharge paperwork and obtain medication scripts from physician.
  • Discharge residents and follow all discharge protocols.
  • Ensures proper documentation in place within PCC.
  • Collaborate with MDS team and other departments/roles as needed.
  • Assist with other tasks as required, such treatments, supervision, dining room supervision, etc.
PHYSICAL REQUIREMENTS OF A RN ADMISSIONS AND DISCHARGES:
  • Requires movement throughout entire unit.
  • Must be able to lift and transport equipment.
  • Must be able to lift and turn residents as needed.
  • Requires ability to keyboard utilizing computerized medical record program.
RN ADMISSIONS AND DISCHARGES

QUALIFICATIONS:
  • Graduation from an accredited school of nursing and current licensure by the State Board of Nursing.
  • Strong Attention to Detail and Work Ethic.
  • Previous Experience in a Skilled Nursing Facility Preferred.
