Licensed Practical Nurse, Home Health

Join Center Well Home Health as an LPN in our home health team. You’ll provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes.

Base pay: $44,600 – $61,400 per year

Pay per visit/unit: $28.00 – $40.00

• Provide professional nursing services under the direction of an RN in compliance with the state’s Nurse Practice Act and Center Well Home Health policies.
• Provide skilled nursing interventions for patients’ illness, rehabilitative needs, and preventive care, using a holistic approach to therapeutic interventions and education.
• Apply clinical practice standards to facilitate problem resolution and achieve individualized patient outcomes.
• Consult with the RN supervisor regarding changes in the plan of treatment; accept verbal orders from physicians permitted by state law and communicate these orders to the RN supervisor.
• Implement a systematic, goal‑driven approach to the nursing plan of care.
• Maintain timely contact with patients, physicians, clinical managers, and other team members regarding patient/family needs and status changes; participate in care coordination and discharge planning as appropriate.
• Maintain the highest standards of professional conduct with confidential information; share information only when legally required and in the patient’s best interest.
• Ensure appropriate communication to obtain supplies and equipment needed for safe implementation of the care plan.
• Prepare, submit, and maintain documentation as required by the company; document shifts and visits in the system.

• Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• At least two years of LPN/LVN experience in a clinical setting, preferably in home health or hospice.
