Position SummaryThe RN Home Admissions facilitates timely access to hospice care by providing comprehensive preadmission and admission services. This role collaborates with internal staff, community partners, and referral sources to overcome barriers to admission and ensures patient and family needs are met with compassion and professionalism. The RN conducts initial assessments, develops immediate care plans, coordinates care transitions, and educates stakeholders on hospice philosophy and eligibility criteria.
Flexibility is required to work across home, long term care and hospital settings.
Employees assigned this job title work understand that the length of the workday is determined by workload. Employees must be able to flex hours as needed to meet referral and admission requests.
Essential Functions- Promote hospice as the preferred provider within the community and healthcare system.
- Educate patients, families, and referral sources about hospice services and eligibility criteria.
- Conduct initial assessments, including physical, functional, psychosocial, and spiritual needs.
- Apply appropriate nursing interventions and document care accurately in the electronic medical record.
- Determine hospice eligibility in accordance with Medicare Local Coverage Determinations and state regulations.
- Coordinate physician communication, obtain admission orders, and initiate medications and DME.
- Develop the initial plan of care and facilitate seamless patient transitions.
- Provide emotional support to patients and families during the admission process.
- Maintain HIPAA compliance and assess safety and risk factors per organizational policies.
- Mentor and educate new staff members and provide in-services as needed.
- Build and maintain strong working relationships with community partners, referral sources, and healthcare facilities.
- Participate in professional development and public education efforts.
- Use resources responsibly and adhere to organizational policies and universal precautions.
- Perform other duties as assigned.
- Knowledge of hospice services, philosophy, and applicable admission regulations.
- Understanding of Advance Directives, Medicaid processes, and healthcare placement options.
- Skilled in pain and symptom control, customer service, and interdisciplinary collaboration.
- Ability to identify and address psychosocial, cultural, and spiritual needs.
- Strong verbal and written communication; capable of managing sensitive conversations.
- Demonstrated clinical judgment, critical thinking, and problem-solving.
- Adaptable to change, able to work autonomously, and manage multiple priorities.
- Competence with electronic medical records and mobile technology.
- Minimum two years of experience in a hospital or long-term care setting preferred.
- Current Registered Nurse license in the State of Florida.
- Current CPR certification.
- Valid driver’s license and acceptable driving record.
- Certification in Hospice and Palliative Nursing (preferred).
- Diploma in Nursing, Associate Degree in Nursing (ADN), or Bachelor of Science in Nursing (BSN) from an accredited program.
- Medical clearance for communicable diseases.
- Compliance with OSHA and CMS vaccination requirements, TB, Hepatitis, and Flu.
- Reliable transportation and ability to work flexible hours and locations.
