About the Role
Licensed Practical Nurse (LPN) – Home Health at Optum, a part of Apex Home Health, LHC Group. This role delivers high‑quality patient care and technical nursing skills in patients’ homes, supporting a caring and collaborative environment.
Primary Responsibilities- Provide clinical services within scope of practice, following state nursing laws, care plans, and coordination with the healthcare team while adhering to infection control procedures.
- Document patient visits comprehensively in the electronic medical record and use the agency’s electronic visit verification program.
- Participate actively in care coordination, report patient needs and changes, attend case conferences, and communicate effectively with patients, families, physicians, clinicians, agency managers, and case coordinators.
- Current, unrestricted LPN licensure in the state of practice.
- Current CPR certification.
- Driver’s license and vehicle insurance or reliable transportation access.
- Ability to work flexible hours to meet client needs.
- Ability to function in any home setting regardless of client age, race, creed, color, sex, disability, or financial condition.
- Home‑care experience.
- Capability to manage multiple tasks simultaneously.
- Independence in working.
- Strong communication, writing, and organizational skills.
Annual total cash target pay: $43,900 – $97,100 (full‑time)
Per visit point: $25.33 – $56.02
Hourly rate: $21.11 – $46.68
Compensation includes visit‑point and hourly pay based on productivity, local labor markets, education, and experience. Benefits include a comprehensive package, recognition programs, equity stock purchase, and 401(k) contribution.
