Primary Purpose :
We are proud of the quality care we provide members, and our team is rapidly expanding to meet the demands of our growing business. We are seeking a highly skilled and compassionate certified Nurse Practitioner to join our Advantmed provider network. In this role, you will conduct in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. Your primary goal is to assess the overall health and well-being of members to ensure accurate and comprehensive risk adjustment coding, leading to improved value-based care.
This role is 1099 and offers flexibility and opportunities for those balancing multiple priorities.

NP Responsibilities :
- Perform annual in-home wellness visits and risk adjustment health assessments on populations with chronic conditions, including comprehensive evaluations such as physical exams, vital signs, medication reviews, functional assessments, cognitive and mental health screenings, and home environment assessments.
- Provide individualized quality care to the elderly population in their homes.
- Deliver patient health education on topics like pain management and medication adherence.
- Build trusting relationships with members through effective communication and empathy.
- Assist in closing care gaps, including screenings and labs.
- Contribute to care delivery improvements through collaboration.
- Commit a minimum of 30 hours per month; this is a part-time position.

Requirements
NP Qualifications :
- Valid, unencumbered NP license for the state of employment.
- Ability to travel within a 55-mile radius from the assigned county.
- Preference for weekday and standard working hour schedules.
- Previous in-home risk assessment experience preferred.
- Experience with 1099, PRN, or part-time roles is a plus.
- At least 3 years of patient care experience preferred (primary care, adult, geriatric, EMR).
- May need to obtain additional NP licensure supported by Advantmed.
- Reliable transportation to travel within designated areas.
- Proficiency with EMR systems.
- Ability to work independently.
- Bilingual skills are a plus.
