Baptist Health is seeking a compassionate and motivated Social Worker II to join the Behavioral Health team at Wolfson Children’s Hospital. This PRN Days position plays a vital role in supporting pediatric patients and their families by addressing psychosocial needs and ensuring access to essential services and resources.

• Assess patients’ psychosocial needs to support emotional well‑being and optimal functioning.
• Provide discharge planning, including coordination of home health services and durable medical equipment (DME).
• Assist with patient placement and transfers, ensuring smooth transitions of care.
• Offer referrals to community resources to support ongoing care and family needs.
• Support utilization review activities to promote effective and appropriate care delivery.

• Master’s Degree (required)
• Doctorate (preferred)

• 1-2 years Counseling Experience - required
• 1-2 years Experience in Child Welfare Services - required
• 1-2 years Social Work Experience - required
• 1-2 years Hospital Experience - required
• Less than 1 year Experience in Behavioral Health Setting - required

• Licensed Clinical Social Worker (preferred)
• Driver’s License (preferred)
