Our programs address substance abuse, HIV care, and primary healthcare, improving quality of life through compassionate and comprehensive care.
OverviewWe are seeking a dedicated Targeted Case Manager to join our team. This role plays a vital part in improving the quality of care for our clients by coordinating services, advocating on their behalf, and promoting satisfaction and cost-effective solutions. The ideal candidate will be responsible for managing the full cycle of client care from assessment through evaluation.
Key Responsibilities- Conduct comprehensive social assessments for all enrolled clients.
- Develop individualized service plans based on assessment findings.
- Coordinate and monitor the delivery of services to clients.
- Link clients to appropriate community resources and advocate on their behalf.
- Assist clients with applications for benefits and support services such as Medicaid, Medicare, SNAP, SSDI, and VA benefits.
- Maintain compliance with Medicaid guidelines and agency standards.
- Meet agency productivity goals by delivering a required number of service units.
- Submit accurate and timely documentation, reports, and case notes.
- Uphold agency policies, procedures, and ethical standards in all client interactions.
- Collaborate with supervisors, co-workers, and community partners to support client needs.
- Bachelor’s degree from an accredited institution in Social Work, Counseling, Psychology, Criminal Justice, Health Education, or a related human services field.
- Minimum of two (2) years of experience working with behavioral health populations.
- Completion of AHCA-approved Case Management Training within three (3) months of hire (or willingness to complete).
- Strong written and verbal communication skills.
- Ability to work independently and as part of a team.
- Must demonstrate professionalism, cultural competence, and respect in all interactions.
- Effective collaboration with community providers and internal teams.
- Strong organizational and documentation abilities.
- Flexibility and problem-solving in a dynamic environment.
- Commitment to representing the agency with dignity and integrity.
