* Must have ATLS updated on 6/6/

Position specific questions

· Reason for the need? New start

· Is there a specific start date needed? ASAP

· What’s the schedule? On-site call coverage weekends nights – MD: 44 hrs/day APP: 42 hrs/day
o Are there specific dates needed?
o If call is involved what is call volume? How often do they get called back on-site?
o Call/Rotation schedule?

· Will they take an APP or SFA when applicable? No

· Who should be the timesheet approver each week? Scheduler Krystal Stewart

· What are expectations for orientation? ie shadow day? EMR training? Orientation at the facility to include EMR additional Shadow Day

· Number of beds? 30-bed ED ­5-bed Fast Track and 2 Trauma Bay Rooms

· Are there any consults and if so how many per day?

· What’s the patient population?

· Trauma center? Types of cases? Acuity? Level 2 Trauma

· What does the support staff look like?

· How will follow up cases be handled? when applicable- inpatient or outpatient

· What does support staff include?

Meditech Pdoc EMRM Modal Voice Dictation Academic focus with IM residents and medical students

General Comments

Must be willing to work at FSED's associated with main facility
