Baptist Health is seeking a compassionate and detail-oriented Registered Nurse, Case Manager to join our In-Patient Case Management Department at Baptist Medical Center Jacksonville Beach. This is a full-time, day shift position.

As a Case Manager, you will play a critical role in coordinating care for hospitalized patients, ensuring smooth transitions through the continuum of care, and working closely with the multidisciplinary team to optimize patient outcomes.

Key Responsibilities:

* Perform high-risk screenings and comprehensive assessments

* Coordinate care plans and post-acute services with accurate documentation

* Facilitate communication among providers, patients, and families

* Support safe and timely discharges through effective resource utilization
