Maplewood Healthcare is a 156-bed facility that offers short- and long-term care, as well as outpatient services. Join a supportive in-house rehab team dedicated to exceptional patient outcomes and meaningful collaboration.
Responsibilities- Review available resident information and evaluate the impact on assessment and treatment.
- Conduct and document thorough functional resident assessments using a standardized procedure.
- Develop and implement individualized treatment plans with achievable functional goals.
- Comply with evaluation, treatment, and documentation guidelines; maintain timely and accurate records.
- Instruct Assistants in treatment procedures and provide supervision while they work with residents.
- Enter treatment and progress notes after each session.
- Revise treatment plans as needed based on resident performance.
- Document the course of treatment in clear, concise discharge summaries.
- Orient residents to program services and treatment procedures appropriate to their needs.
- Collaborate with nursing staff, residents, and families to maximize functional abilities.
- Educate residents and caregivers on therapeutic exercises and procedures for home continuation.
- Assist with referral processes for additional services post-discharge.
- Attend and participate in rehabilitation team conferences and other facility meetings.
- Access continuing education opportunities to maintain clinical competency.
- Participate in quality improvement activities.
- Adhere to Ahava Therapy Services’ regulatory and professional standards.
- Follow facility policies and participate in safety programs.
- Perform other duties as assigned.
- Understand reimbursement guidelines and allocate therapy resources efficiently.
- Maintain productivity levels and support the company's mission and vision.
- Bachelor’s or Master’s degree in Physical Therapy
- Current Tennessee PT license
- Current BLS/CPR certification
- Strong observational, verbal, and written communication skills
- Ability to work collaboratively with an interdisciplinary team
