Wraparound Facilitator

Overview

A Wraparound Facilitator/Specialist provides comprehensive, individualized care coordination for youth and families, building a support team to address academic, social, emotional, and safety needs. They create and implement personalized care plans, facilitate meetings, and connect families with community resources to foster long-term stability and success.

• Case Coordination:
  Develops and manages individualized, strength-based plans in collaboration with families, schools, and community partners.
• Team Facilitation:
  Convenes and leads Child and Family Team (CFT) meetings to identify needs and create safety/crisis plans.
• Support & Advocacy:
  Provides outreach, advocacy, and direct support to help families navigate complex systems (mental health, housing, etc.).
• Monitoring & Evaluation:
  Implements the plan, tracks progress, and adjusts strategies to meet goals.
• Documentation:
  Maintains accurate, confidential records in compliance with agency and legal standards.

• Education:
  
  Bachelor’s degree in Social Work, Psychology, or related field (often required), or equivalent experience.
• Experience:
  
  Experience working with children, youth, and families with complex needs, preferably in a case management or social service role.
• Skills:
  
  Strong communication, organizational, problem-solving, and de-escalation skills.
