Licensed Practical Nurse LPN Clinical Coordinator

As a LPN Clinical Coordinator, you’ll contribute to our success in the following ways:
  • Review and clear HCHB coordination notes and process administrative task workflows in an accurate and timely manner.
  • Obtain orders from physicians, including verbal, written, and faxed orders, and enter into appropriate system for CTM review and approval.
  • Monitor patient hospitalization list, confirm status, and coordinate appropriate resumption of care.
  • Discharge patients in inpatient facility at end of episode (EOE).
  • Obtain patient-related attachment renewal.
  • Participate in High-Risk Meetings and ensure that the best provision of services is offered.
  • Available to provide direct care, as needed.
  • Answer telephone calls from patients, patient representatives, and medical professionals and communicate messages to the appropriate party or parties.
  • Provide clerical support to patients such as scheduling follow-up appointments, providing health summaries to case managers, obtaining pre-authorizations, and addressing patient questions or concerns.
What is Required?
  • Graduate of an accredited school of professional nursing.
  • Current Certified Licensed Vocational Nurse or Licensed Practical Nurse.
  • 1 year of home care or hospice experience.
  • Able to seek patient feedback, advocate for patients, understanding of patient needs, and managing patient expectations.
  • Able and willing to travel up to 50% as needed to support business and patient needs.
  • Hold ability to sit, stand, bend, lift, and move intermittently and be able to lift 50-100 lbs.
  • Valid driver’s license and insurance and reliable transportation to perform job tasks.

You will report to the Clinical Team Manager.
