Our vision is to enhance the lives of seniors and their families by reaching beyond our walls through community partnerships and innovation. We’re seeking Nurses to join our team!

• Assess and evaluates patient’s needs for, and responses to, care rendered
• Apply sound nursing judgment in patient care management decisions
• Provide primary and emergency care for occupational and non-occupational injuries and illnesses
• Administer over the counter and prescription medication as ordered
• Collaborate with the nursing team to create Plan of Care for all patients
• Coordinate self-driven wellness programming for those individuals who are more independent, and less reliant on assistance
• Direct and guide ancillary personnel and maintain standards or professional nursing
• Coordinate healthcare services (specifically those services that come from sources outside of the health center facility — like on-boarding and supporting hospice staff)
• Build and maintain relationships with other home health organizations
• Maintain consistent contact with local physicians to stay up-to-date on each individuals’ medical needs and service expectations
• Coordinate discharges for individuals whose needs are no longer being met

• Valid RN or LPN license required, will consider graduated nurses
• Advance knowledge in EMR’s
• Willingness to work flexible schedule and to fill in when needed
• Excellent bedside manner and communication skills
• Working knowledge of health care laws and regulations
