Optimal Care is a clinician-owned and operated company delivering home health, physician services, and hospice care. This role is based in Jackson, MI, serving Jackson and surrounding areas.
Responsibilities- Provides physical therapy services to patients according to a written individualized plan of care.
- Assesses and evaluates therapeutic/rehabilitative/functional status and participates in the development of the total plan of care. Evaluates the home environment.
- Assesses muscle strength, mobility, gait, ROM, and transfer capabilities; aims to improve or minimize residual disability.
- Provides physical therapy as prescribed by a physician or advanced practice practitioner.
- Educates patients on equipment required to increase function and independence and return to the patient s optimal and productive level within their capability.
- Possesses a degree from an accredited Master’s or doctoral program as a Physical Therapist.
- 1-year clinical experience as a Physical Therapist preferred.
- Interpersonal and professional communication skills with staff, patients, and caregivers.
- Knowledge of medical terminology.
- Problem-solving, organizational skills, and attention to detail.
- Strong verbal and written communication skills.
- Ability to handle multiple priorities, complete documentation, and meet deadlines.
- Reliable transportation and valid automobile insurance.
- Sit, stand, bend, lift, and move intermittently; ability to lift at least 35 lbs.
- Home health experience preferred.
- Familiarity with Home Care Home Base (HCHB).

8:00 AM – 5:00 PM, Monday – Friday
