Optimal Care is hiring a Speech Language Pathologist (SLP). A Speech Language Pathologist administers speech therapy to patients on an intermittent basis in their place of residence, in accordance with physician orders and plan of care under the supervision of management. Services are furnished only by or under the supervision of a qualified speech pathologist or audiologist.
Responsibilities- Provide a full range of Speech Language Pathology services as ordered by physician
- Periodically participate with other home care personnel in patient care planning
- Take initial history and perform initial evaluation; administer skilled procedures
- Consult with physicians regarding changes in treatment and report patient progress
- Complete an initial evaluation with plan of treatment and goals; obtain physician approval; recertify every 60 days if treatment continues; write progress notes each visit; reevaluate when duration of treatment ends; write a summary upon discharge
- Master's or Doctoral degree in Speech Language Pathology and licensure as a Speech Language Pathologist by the state of service
- Knowledge of medical terminology
- Reliable transportation and valid automobile insurance
- Two (2) years' experience as a Speech Therapist;
Home Health experience preferred
