Optimal Care is a clinician-owned and operated company delivering home health, physician services, and hospice care. This role is based in Jackson, MI, serving Jackson and surrounding areas.

• Provides physical therapy services to patients according to a written individualized plan of care.
• Assesses and evaluates therapeutic/rehabilitative/functional status and participates in the development of the total plan of care. Evaluates the home environment.
• Assesses muscle strength, mobility, gait, ROM, and transfer capabilities; aims to improve or minimize residual disability.
• Provides physical therapy as prescribed by a physician or advanced practice practitioner.
• Educates patients on equipment required to increase function and independence and return to the patient s optimal and productive level within their capability.

• Possesses a degree from an accredited Master’s or doctoral program as a Physical Therapist.
• 1-year clinical experience as a Physical Therapist preferred.
• Interpersonal and professional communication skills with staff, patients, and caregivers.
• Knowledge of medical terminology.
• Problem-solving, organizational skills, and attention to detail.
• Strong verbal and written communication skills.
• Ability to handle multiple priorities, complete documentation, and meet deadlines.
• Reliable transportation and valid automobile insurance.
• Sit, stand, bend, lift, and move intermittently; ability to lift at least 35 lbs.

• Home health experience preferred.
• Familiarity with Home Care Home Base (HCHB).
