Geriatric Home Health LVN Amcare Pro Home Health is a mid-size home health care company with offices in DFW & East Texas. We are currently interviewing for a PRN LVN Field Nurse to cover the Irving, Carrollton, Coppell, Farmers Branch and surrounding areas.
We are looking for a Home Health LVN to provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse. This role supports the delivery of safe, high-quality patient care through routine nursing visits, treatment administration, patient education, and timely documentation. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/AXXESS) is a plus.
Job Responsibilities- Provide skilled nursing care to home health patients under the supervision of an RN
- Perform routine nursing visits including vital signs, medication administration, injections, and treatments
- Assist with wound care and dressing changes per physician orders
- Monitor patient condition and promptly report changes to the supervising RN
- Educate patients and caregivers on medications, treatments, and safety
- Accurately document all visits and care provided in the agency EMR
- Ensure compliance with Medicare, Texas regulations, and agency policies
- Coordinate visits and communicate effectively with the clinical team
