Experience/Qualifications
  • Licensed Vocational Nurse (LVN) certification in good standing.
  • 2 years Clinic Experience
  • Proven experience in care coordination within a healthcare setting.
  • Commitment to continuous professional development and learning.

  • Ability to analyze patient data and adjust care plans accordingly.

Responsibilities

  • Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
  • Collaborate with multidisciplinary teams to optimize patient care and outcomes.
