Position: Community Health Worker I

Job Summary

The Community Health Worker 1 delivers community health services in the areas of needs assessments, referral services, client education, documentation, and diversity education.

Location:

Irving, TX

Schedule:

Full Time – Monday to Friday (1 to 2 weekend days required)

Essential Functions of the Role
  • Assists clients in gaining access to needed services and increasing health knowledge and self‑sufficiency while learning how to best utilize available tools and services (e.g., outreach, patient navigation, follow‑up, community health education, informal counseling).
  • Assists with referrals and linkage to follow‑up services within the community and within the Organization.
  • Assists with needs assessments and the development of patient plans.
  • Conducts culturally appropriate skills‑building self‑management education sessions on different topics for groups and individuals, building understanding of ethnicity, language, socioeconomic status, and life experiences.
  • Teaches basic concepts of health promotion, disease prevention, and self‑management.
  • Obtains basic vital signs, measures height and weight, and conducts point‑of‑care testing.
  • Administers evaluation tools to assess program effectiveness.
  • Documents plans, goals, education, and follow‑up in a timely manner using agreed‑upon documentation standards.

  • Teaching skills preferred.

  • Ability to administer evaluation tools to assess program effectiveness.
  • Ability to conduct needs assessments, develop patient plans, and navigate patients to appropriate interventions.

Qualifications
  • Education:

    H.S. Diploma/GED Equivalent.
  • Experience:

    Less than 1 Year.

Certification / License / Registration – Community Health Worker (CHW): CHW license by the State of Texas.
