Full Time, Monday - Friday (8:00am - 5:00pm)

- Graduate of an accredited school of Advanced Practice Registered Nursing.
- Completion of End of Life Nursing Education Program (ELNEC) – all 8 modules within 18 months of employment.
- Current Advanced Practice Registered Nurse License within the state of employment.
- MSN or Doctoral Degree required.
- Certification in Hospice and Palliative Nursing desired.
- Valid driver’s license with a good driving record; compliance with all state and federal driving regulations.
- Reliable transportation and willingness to drive to patient locations.
- Ability to lift 50 pounds.
- Must clear a criminal background check, physical and drug screening.
- May be required to work outside regular hours depending on business needs.
- Assess signs and symptoms of discomfort, including presence of pain, and take prompt and appropriate action.
- Initiate plan of care and provide all services to patients/families in all settings.
- Provide direct face‑to‑face visits to referred and existing patients; contact physicians for patient updates.
- Maintain knowledge of state and federal regulations and JCAHO standards (if applicable).
- Participate in the QAPI program.
Anticipated pay range: $75,000 - $80,000 USD.
