Manager of Mental Health & Addictions and Quality Assurance Auditor
Reporting clinically to the Manager of Mental Health and Addictions, and administratively to the Quality Assurance Auditor, provides comprehensive social worker services and therapeutic clinical counseling to patients and families for the assigned clinical specialty area.
Responsibilities- Conducts comprehensive social work and psychosocial assessments of patients and families by interviewing the patient and/or family, obtaining relevant information, gathering social data, and interpreting the data using tools in accordance with professional standards, clinical policies and practices.
- Provides comprehensive therapeutic clinical services such as grief and bereavement counseling to patients and families; collaborates with patients, families, community supports and the interdisciplinary team to coordinate services, manages cases and facilitates meetings.
- Provides consultation and education to patients, families and healthcare professionals on issues related to high risk factors, recovery and current practice in areas such as illness and disability, trauma and bereavement, critical incident stress management, protection of vulnerable adults, chemical dependency, community living and support services.
- Maintains patient records by documenting assessments, charting information, preparing progress notes and reports in accordance with established standards and policies to comply with regulatory requirements.
- Provides leadership to the interdisciplinary team in care planning (e.g., discharge planning) by assessing patient/family needs and communicating care plans to the team.
- Arranges meetings on patient admissions to develop and coordinate care plans and community services for patients requiring facility placement and/or home support; collaborates with hospital and community services to determine alternate level of care and resource utilization.
- Communicates with community health and social agencies, including provincial Ministries, Home Health personnel and family physicians, by coordinating assessments and management and referring to appropriate resources when longer-term treatment or follow-up is required.
- Acts as a clinical resource by providing information regarding community resources and facilitating access to those resources.
- Participates as a member of local committees and planning bodies to evaluate programs, identify opportunities for change, assess resources, and improve efficiency and effectiveness in service delivery.
- Maintains a variety of records such as workload statistics, analyzes trends, and prepares reports, contributing to action plans as required.
- Develops, coordinates and delivers in-services, presentations, psycho-educational and support groups, and collaborates with interdisciplinary team to plan, plan content, and coordinate training activities.
- Initiates and conducts research activities and program planning relevant to patient, family, hospital and community needs, aligning with current trends and literature.
- Maintains professional development through literature review and collaboration with clinical and community experts to improve care.
- Works as an integral member of the team including ALC nurse, Resident, Client, Patient Experience and Utilization, Health Systems Navigator.
- Performs other related duties as assigned.
Education and Experience
- Master's degree in Social Work from an approved school of Social Work, plus one (1) year recent, related acute care clinical experience.
- Current full registration with the Ontario College of Social Workers.
- Applicants with a Bachelor of Social Work (BSW) from an accredited School of Social Work and a demonstrated equivalent combination of education, training and experience may be considered. If applying under equivalency, please outline in your application how your knowledge, education, skills and experience meet a Master of Social Work (MSW) equivalency.
Skills And Abilities
- Comprehensive knowledge and understanding of emotional and mental health issues related to illness, injury and patient/family cultural dynamics.
- Comprehensive counseling skills including crisis counseling, brief therapeutic interventions and loss and grief applicable to patients, families and their support networks.
- Ability to educate and provide consultation to patients, families and healthcare professionals.
- Ability to develop, coordinate and deliver in-services, presentations, psycho-educational and support groups.
- Ability to provide leadership to the interdisciplinary team in care planning for effective utilization of patient resources.
