- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
- Current and unrestricted license to practice medicine in Ohio.
- Current DEA registration and BLS certification required.
- Board certification preferred but not required.
- Minimum 3–5 years of clinical experience.
- Prior leadership experience in an FQHC or similar environment preferred.
- Experience in integrated care settings (medical, dental, behavioral health).
- MAT experience preferred but not required.
- Open to all physician specialties that support primary care, population health, and FQHC operations (not limited to Family Medicine).
