Barrack Lane Medical Centre has an opportunity for a care navigator to join our friendly, supportive and professional team on a full-time basis, 37.5 hours per week.
The care navigator is a very important role within the surgery as they are the first person a patient speaks to or sees when they contact the surgery.
Main duties of the job- Greeting and directing all patents and handing out prescriptions, letters, completed forms, and specimen bottles.
- Making appointments face-to-face, online and via the telephone.
- Answering the telephone promptly and politely.
- Managing the needs of the patient, dealing with enquires and advising of practice procedures.
- Helping patients access the right services.
- Ensuring all information is recorded accurately on the computer system.
- Working as part of the care navigator team and supporting all members of the practice staff.
Barrack Lane is a busy practice in the town centre of Ipswich, Suffolk. We have a practice population of over 20,000 patients. The practice is based on two sites, the main surgery in the centre of the town, and a branch surgery at Stoke Park.
Barrack Lane is part of a PCN (Primary Care Network) with Ivry Street Medical Centre, in Ipswich, and the two practices cover a population of 32,000 patients. The PCN allows the practices to share resources and offer an enhanced service to our patients. The PCN employs care co-ordinators and social prescribers to work with patients, especially our vulnerable patients, to help them to access the best care.

Person Specification Experience- Excellent communication skills and a caring approach.
- A clear and polite telephone manner.
- The ability to work calmly and efficiently in a fast paced environment, working well under pressure.
- The need to maintain confidentiality all times.
- A high level of attention to detail.
- The ability to work flexibly to cover colleagues holidays and occasional sickness.
