Schedule: Monday – Friday
Salary: $60,000 – $75,000 per year
Summary:
The facility is seeking a Licensed Independent Social Worker (LISW) to support the mental and emotional well-being of individuals and families in a rural healthcare setting. The LISW will conduct psychosocial assessments, develop treatment plans, and provide therapeutic interventions for patients experiencing behavioral health challenges. This role works closely with an integrated care team to deliver coordinated, patient-centered care.
Responsibilities:
- Conduct comprehensive psychosocial assessments for patients across the lifespan
- Develop and implement individualized treatment and care plans
- Provide counseling and evidence-based therapeutic interventions
- Address behavioral health concerns such as depression, anxiety, trauma, substance use, and adjustment disorders
- Collaborate with physicians, nurses, and behavioral health professionals to coordinate patient care
- Maintain accurate and timely clinical documentation
- Provide patient education, crisis intervention, and referrals to community resources as needed
Requirements:
- Master’s degree in Social Work (MSW) from an accredited program
- Active Iowa Licensed Independent Social Worker (LISW) license
- Minimum 2 years of clinical experience preferred
- Experience working with diverse patient populations and behavioral health conditions
- Strong interpersonal, organizational, and communication skills
- Ability to work effectively in a rural or underserved healthcare setting
