Home Health Therapist

Boost Home Healthcare - Summerlin seeking compassionate and skilled Therapists (PT, ST, OT & MSW) on a per diem basis to join our Home Health team for our 2026 opening! The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong background in various therapeutic specialties, including occupational therapy, speech therapy, physical therapy and MSW.

Therapists will work collaboratively with a multidisciplinary team to manage individualized care plans tailored to each patient's needs. The EMR utilized by our company is Kan Time.

• Conduct comprehensive assessments of patients' health status and needs in their home environment.
• Evaluate and manage individualized nursing care plans based on patient assessments.
• Administer medications and treatments as prescribed by physicians.
• Provide education and support to patients and their families regarding health management and disease prevention.
• Monitor patients for changes in condition and respond appropriately to emergencies.
• Maintain accurate documentation of patient progress and interventions in compliance with regulatory standards.

• Current valid license in the State of Nevada.
• Previous experience in home health care or a related field.
• Proficiency in critical care or urgent care is highly desirable.
• Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams.
