Chaplain

Position Overview:

PACHS is seeking a compassionate and dedicated Hospice Chaplain to provide spiritual counseling to hospice patients and their families. This position focuses on offering emotional and spiritual support while respecting individual beliefs. The Hospice Chaplain will also work closely with the interdisciplinary team to help develop and implement personalized care plans.

• Provide spiritual counseling to hospice patients and their families while respecting their belief systems.
• Maintain confidentiality of patients and their families at all times.
• Document progress notes in the clinical record in a timely manner.
• Use effective communication skills to support patients and families.
• Attend IDT (Interdisciplinary Team) meetings and provide input into the development, implementation, and evaluation of care plans.
• Report any necessary findings to the hospice nurse as needed.
• Assist in arranging visits from clergy or religious community members for patients who request them.
• Be available to families or individuals who do not have a spiritual support system and request services.

• Ordained and endorsed minister, priest, or member of a religious order, or a layperson with certified training in pastoral care.
• Experience in pastoral services and counseling individuals with terminal illnesses.
• Comfortable working with a variety of spiritual and religious practices.
• Strong communication skills and the ability to work well as part of a team.
