Certified Occupational Therapy Assistant (COTA) – PRN

Good Samaritan is seeking a PRN Certified Occupational Therapy Assistant to work at the Forest City Skilled Nursing Facility. The position supports patients with orthopedic, balance/falls/gait, cervical/back pain, deconditioning and other conditions.

Shift & Schedule

Day shift, PRN schedule. Weekly hours vary.

Salary

$24.00 – $38.50 per hour

Job Summary

Assists patients/residents/clients with therapy activities and exercises scheduled in a treatment plan, under the supervision of an occupational therapist. Provides, monitors and records therapy services to patients/residents/clients with physical, cognitive, developmental and psychosocial impairments. Selects, implements, and modifies therapeutic activities consistent with patient goals and practice setting requirements. Works in post‑acute, long‑term care, and home health environments.

Responsibilities
  • Assist clients per scheduled treatment plans.
  • Track and document therapy progress and outcomes.
  • Collaborate with occupational therapists and facility staff.
  • Maintain knowledge of therapeutic outcomes and provide documentation.
  • Follow accepted clinical practice guidelines.
  • Perform additional duties as requested by the team.
Qualifications
  • Appropriate education level required for state licensure.
  • Licensed as a Certified Occupational Therapy Assistant in the state(s) of practice.
  • Preferred certification through NBCOT for centrally located facilities.
  • Valid driver’s license may be required.
  • BLS certification required within six months of employment.
