Physical Therapist – PRN

This position is PRN covering the Skilled Nursing Facilities of Good Samaritan Algona and Forest City.

Salary Range: $37.50 – $62.00 per hour
- Plan, organize and deliver individualized physical therapy programs to help clients recover mobility, relieve pain, increase strength and prevent deformities.
- Conduct examinations, evaluations, and interventions for patients affected by injury or disease.
- Apply tools and techniques to administer physical therapy interventions while following safety safeguards.
- Document patient information, establish and modify therapeutic plans as needed.
- Educate patients and their families about treatment plans, schedules, and self-exercises for home use.
- Incorporate current research and age‑related treatments into daily practice.
- Demonstrate documentation that meets departmental guidelines and regulatory requirements.
- Adhere to professional clinical practice guidelines and maintain flexibility in variable work schedules.
- Collaborate with providers, case managers, third‑party payers, public organizations, and contract partners.
- Supervise assistants, aides, and students when providing patient care.
- Provide ongoing education and training to peers as directed by leadership.
- Perform other duties as requested by management and focus on post‑acute, long‑term care, and home health settings as needed.
- State licensure as a Physical Therapist in the state of practice.
- Valid driver’s license (when applicable).
- Basic Life Support (BLS) certification for health care providers (required within first 60 days).
- Academic education appropriate for state licensure.
- Competency in department‑specific certifications and procedures.
