The Skilled Rehab team at Good Samaritan Algona and Forest City seeks to hire a PRN Certified Occupational Therapy Assistant to join our team.

We are transitioning these locations to an in-house therapy model from contract use. New grads are welcomed. We are looking to hire a candidate who can complement our team. In-house therapy model provides one team, individualized treatment plans, improved collaboration, and a workplace culture that encourages doing good for those we serve.   • Job Summary

    Assists patients/residents/clients with therapy activities and exercises scheduled in a treatment plan, under the supervision of an occupational therapist, to improve quality of life and ability to perform daily activities. Provides, monitors, and records therapy services to patients/residents/clients with physical, cognitive, developmental, and psychosocial impairments in line with related standard concepts, practices, and procedures. Selects, implements, and modifies therapeutic activities consistent with competency levels, patient goals, and state licensure requirements.

    Contributes by exchanging information and documentation with the occupational therapist about patient responses. Responsible for being knowledgeable about targeted occupational therapy outcomes and for providing related documentation. Follows accepted clinical practice guidelines. Performs other related duties as requested. Depending on location and facility needs, work may focus on post-acute, long-term care, and home health environments.

    Requirements

    Must be licensed in the state(s) of practice as a Certified Occupational Therapy Assistant. When applicable, may require a valid driver’s license. Obtain and maintain required department-specific competencies and certifications. Basic Life Support (BLS) for healthcare providers required within the first 60 days of employment.

    Qualifications
    • Appropriate education level required in accordance with state licensure.
    • Licensed as an Occupational Therapy Assistant in the state(s) of practice.
    • If working at a Sanford Health Facility (not required but preferred for GSS Facilities):
      Must also be certified through the National Board for Certification in Occupational Therapy (NBCOT).
    • Conforms to the supervision and licensing standards of the state(s) where services are provided.
    • Valid driver’s license when applicable.
    • Basic Life Support (BLS) certification required within six months of employment.
