As a MDS Coordinator you will:

• Develop, monitor, audit, and modify each resident’s care plan to meet their individual needs and goals.
• Perform resident assessments and assist with the discharge process.
• Provide your team with the knowledge and skills needed to deliver the high standard of care we are known for.
• Maintain accurate and up‑to‑date documentation in compliance with regulatory requirements.

• Unencumbered State of Iowa RN license.
• Graduate of an accredited nursing program.
• CPR certification.
• Preferred: one year experience as an L.P.N./R.N. in a Skilled Nursing Facility and one year of experience as a C.N.A.; prior MDS Coordinator experience is also preferred.
• Willing to invest in the right candidate.
