Country Lane Manor, a skilled nursing facility located in the heart of Van Buren County, is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team on a PRN basis. Our mission is to help residents live life to the fullest by providing care rooted in respect, dignity, and excellence.

Shift: PRN

Rate: Up to $70/hour

Position Summary

As a Physical Therapist at Country Lane Manor, you’ll play an essential role in helping residents regain strength, mobility, and independence. You will evaluate, design, and implement individualized treatment plans that support recovery and improve overall quality of life.

• Deliver high-quality, resident-centered physical therapy care in accordance with professional standards and best practices.
• Evaluate residents and develop personalized treatment plans with measurable short- and long-term goals.
• Collaborate with interdisciplinary team members to promote optimal outcomes and holistic resident well‑being.
• Accurately document assessments, progress notes, and care plans in compliance with facility and regulatory guidelines.

• Licensed Physical Therapist (PT) in the state of Iowa
• BLS Certification (current)
• Passion for working with seniors in a skilled nursing or post-acute care setting
• Strong communication, organization, and interpersonal skills
