Position Title

Weekend Baylor Registered Nurse (RN)

FLSA Classification: Exempt

Position Summary

The Weekend Baylor Registered Nurse (RN) is responsible for partnering with the Clinical Team to administer and coordinate skilled nursing care to patients requiring home care services. Services are provided in accordance with physician’s orders, under the direction and supervision of the Clinical Director, and in compliance with applicable laws, regulations, and policies of the organization.

Schedule: The Weekend Baylor RN’s schedule is every weekend (Saturday & Sunday) from 7:00 AM – 8:00 PM, with four (4) hours on Mondays to conduct follow-ups with providers and/or complete documentation. This is a full-time, exempt (salary) position.

Responsibilities

Clinical Care:

  • Provides direct care and case management of assigned patients, including initial assessment and development of plan of care, as well as oversight and administration of prescribed medications and treatments. Maintains and updates schedules of all disciplines in patient’s home.
  • Assesses and evaluates patient status and goals; revises the plan of care as necessary and communicates adjustments to the team in a timely manner. Complete authorization modifications as needed.
  • Initiates and applies appropriate preventative, therapeutic, and rehabilitative procedures and maintains communication regarding patient care with the physician, supervisory personnel, and other professional staff involved in patient care.
  • Monitors and manages patient medications; maintains an accurate medication profile; coordinates prescription refills with physicians and pharmacies.
  • Educates patient and family/caregiver on self-care, medications, and ordered instructions; provides health counseling opportunities when appropriate.
  • Maintains a safe patient environment and reports unsafe conditions, complaints, and incidents.
  • Observes and supervises Home Health Aide & LPN to evaluate performance and quality of care.
  • Accepts responsibility for specialized procedures (e.g., IV therapy) when qualified and trained.
  • Uses supplies and equipment efficiently, and maintains equipment per agency policy.
  • Manages patient caseload including follow-up with other facilities as needed.
  • Adheres to HIPAA laws and maintains patient confidentiality.
  • Performs other duties assigned by the case manager/clinical management.

Documentation:

  • Completes and submits accurate, timely clinical notes including all OASIS documents, visit authorization requests, physician orders, and related documentation in accordance with Innovive policies.

Communication and Meetings:

  • Communicates with physicians and other agencies to ensure continuity of care and a comprehensive care plan.
  • Participates in clinical and case management meetings; documents all meetings and outcomes in the patient record.
