UI Heart and Vascular Center is seeking a 100% Nurse Clinician to support our Pulmonary Hypertension Program.
UI Health Care is home to the most comprehensive advanced heart failure program in Iowa. The Nurse Clinician within the Advanced Heart Failure Program plays a pivotal role in providing comprehensive care and support to patients facing complex cardiac conditions. This role combines clinical expertise, patient education, and coordination of care to optimize patient outcomes and enhance their quality of life.
The Nurse Clinician collaborates closely with multidisciplinary teams, including physicians, nurse practitioners, social workers, and other healthcare professionals, to develop and implement individualized care plans tailored to each patient’s unique needs. Responsibilities include conducting thorough patient assessments, managing symptoms, administering treatments, and offering guidance on medication management and lifestyle modifications. Additionally, the Nurse Clinician serves as a primary point of contact for patients and their families, offering emotional support, education, and resources throughout the continuum of care.
Through their dedication and compassion, Nurse Clinicians within the Advanced Heart Failure Program play a vital role in improving patient outcomes and fostering a culture of excellence in cardiac care.
- Collaborate with nursing, medical staff and other health professionals in the development and review of standards and in planning, implementing, and evaluating patient care for the patient population with regard to the condition and age of the patient. Provide direct nursing care when necessary.
- Develop and implement individualized care plans in collaboration with the multidisciplinary team, tailored to each patient’s specific needs and goals.
- Monitor and manage symptoms of advanced heart failure, including dyspnea, edema, fatigue, and chest pain.
- Administer medications, including intravenous and subcutaneous therapies, and provide patient education on medication management, potential side effects, and adherence.
- Oversee patient flow through the UI Health Care continuum of care on both the inpatient and outpatient setting.
- Facilitates care of patient(s) requiring admission, coordinating necessary admission procedures, pre and post procedure care.
- Provide patient education in the HVC that includes:
- Coordinate and facilitate patient education sessions on pulmonary hypertension management, self-care strategies, and lifestyle modifications to promote optimal health outcomes.
- Facilitating learning and provides consultation to those in external agencies, communities and to other health care professionals.
- Supporting implementation and evaluation of staff education and orientation programs, coordinating and presenting.
- Facilitating the educational experience for health care students and monitors the care delivered.
