Overview
The University of Iowa Health Care, Department of Pharmaceutical Care is seeking a 100% Clinical Pharmacist to work in our Ambulatory Care Pharmacies
. The Ambulatory Care Clinical Pharmacist provides pharmaceutical care to patients seen in UI Health Care Ambulatory Care Pharmacies and works closely with pharmacy staff and social workers on the PMAP team to provide medication financial assistance to patients. The pharmacist provides clinical support, education, information related to medication costs and alternatives, and dispensing and monitoring of patients receiving medications through UI Health Care Ambulatory Pharmacies.
The pharmacist will be responsible for appropriate screening and review of patients, documentation, and follow up communication to the patient and other impacted teams. A sign-on bonus up to $10,000 is available for selective candidates.
- Provides Therapeutic Drug Monitoring (TDM) on clinic-administered medications and take-home prescription orders.
- Completes a comprehensive patient assessment including evaluation of the patient’s medications and medical history.
- Evaluates all prescription orders to ensure the appropriateness of: indication for use, dose, route of administration, drug interactions, dose adjustments for organ dysfunction, physical and therapeutic incompatibilities, drug-induced side effects and adverse reactions, potential drug misuse or abuse, use of the most cost-effective therapy, and duplicate therapy.
- Ensures pharmacokinetic assessment of drug dosing on targeted patients through clinical assessment and preparation of formal dosing recommendations.
- Communicates with prescribers about patient medication needs.
- Conducts review of patient profiles and updates as needed based upon findings at time of patient visit.
- Provides appropriate drug information to other healthcare professionals.
- Assesses drug allergies noted in the medical record.
- Participates in collecting and documenting medication histories on patients.
- Evaluates patient compliance with prescribed medication therapy and identifies problems that may lead to partial or noncompliance.
- Utilizes appropriate resources and coordinates the resolution of financial concerns with department financial counselors.
- Provides an evaluation of medication costs and options to support affordable care to patients.
- Completes benefits investigations for prescriptions sent to the Medication Assistance Center.
- Evaluates patient’s prescription coverage status and provides options or referrals as necessary.
- Investigates options for copay assistance including copay cards, grants, manufacturer programs, repository medications, and Charity Care.
- Works with PMAP social workers to screen and enroll patients in appropriate programs, as applicable.
- Communicates with prescribers, pharmacy teams, and other clinical staff about patient medication needs.
- Appropriately labels, tracks, and monitors inventory and supply in the pharmacy.
- Accepts shipments from manufacturer programs and documents receipt.
- Labels medications with required regulatory information prior to dispensing.
- Maintains and tracks inventory brought into the pharmacy.
- Communicates about dispenses through other pharmacy pickup or clinic locations.
- Documents all patient care activities and maintains records in the patient medical record and other online systems, including: patient drug allergies, medication histories, counseling, interventions with prescribers, patient interactions, dosing recommendations, and Drug Use Evaluation activities.
- Educates patients on the pharmaceutical care services offered, including insurance copays, financial assistance options, medication history, allergy history, and adherence strategies.
- Provides patient teaching and coordinates information with local healthcare providers to ensure continuity of care.
- Acts as a patient advocate and participates in hospital-wide programs to improve care (e.g., Drug Use Evaluation Program, Adverse Drug Reaction Reporting).
- Performs Medication Use Evaluation initiatives through prescriber intervention and education.
- Serves on intra- and interdepartmental committees, task forces, and teams as required.
- Provides educational services including serving as a preceptor for pharmacy students/residents and presenting in-service programs.
The ambulatory care clinical pharmacist facilitates transitions from inpatient to outpatient settings, including bedside delivery of discharge medications and applying appropriate education at the point of care to expedite discharge. Complete and maintain compliance with all competencies and educational requirements.
Qualifications- Education Requirements: Pharm
D (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy. - Required: Current licensure by the Iowa Board of Pharmacy Examiners;
