The University of Iowa Healthcare department of Care Coordination is seeking a Social Worker III to provide direct social work services to the Emergency Department/Psychiatry patients. Responsibilities include assessment, treatment, interventions, and recommendations to facilitate movement through the continuum of care for patients and their families. This role will also participate in interdisciplinary activities, providing the social work perspective and treatment to patients, clients, and families with complex emotional or social problems.
Dutiesto Include
- Develop and implement a plan of care responsive to patient/family concerns, healthcare status, available resources, age‑specific problems, and clinical impressions.
- Identify patients requiring social work intervention through case finding.
- Perform social work assessments with patient and/or family involvement.
- Develop clinical impressions and rate severity of problems for planned interventions.
- Provide a range of social work services responsive to identified problems (e.g., help with psychosocial functioning, adaptation to illness, facilitating services, protective services, post‑hospital planning).
- Provide information, consultation, and professional support to interdisciplinary team members.
- Share information with team members regarding psychosocial issues and their impact.
- Inform team members of social work treatment goals, patient status, progress, and collaborate on service provision.
- Act as a resource for the team (participate in patient care rounds, team conferences).
- Advocate for the social work role with healthcare team members.
- Use social work knowledge to remedy team‑related issues and help evaluate and enhance team functioning.
- Contribute to strengthening the social work profession.
- Contribute to the development, strengthening, and delivery of needed health and social services.
- Participate in departmental and unit‑based activities.

100% of time. Scheduled hours: 2:00 pm – 11:30 pm. Emergency Department social work coverage is 24/7, split into 12‑hour shifts, with potential for weekend and holiday coverage.
Pay GradePay Grade: SEIU Pay Plan

- Master’s degree with a major concentration in social work.
- 6 months–1 year of clinical social work experience.
- A license to practice social work in Iowa.
- Excellent written and verbal communication skills.
- Some familiarity with social work undergraduate education.
- Social work experience in a healthcare setting with the appropriate patient population and their families.
- Experience with identifying and navigating financial resources—including insurance coverage options and changes related to federal and state health programs, and the disability process.
- Experience working with community resources, home healthcare, nursing facilities and other outpatient options.
- Experience in a fast‑paced, high‑volume work environment.
- Care coordination experience or case management skills in a healthcare setting as part of an interdisciplinary team, including experience using an electronic medical record to manage non‑adherence, post‑care follow‑up, and referrals to resources.
