Overview
Medical Social Worker (MSW / BSW) Emotional support. Resource guidance. Advocacy. Support patients and families through compassionate guidance, emotional support, and meaningful connection during the hospice journey. The Medical Social Worker provides psychosocial support to hospice patients and families receiving care in home or facility settings, helping address emotional, social, and practical needs throughout the course of care.
Schedule, Hours & Territory- Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM
- Territory:
Lansing, MI with occasional support in the Grand Rapids market - Participation in an on-call rotation may be required with additional compensation provided

- Conduct psychosocial assessments
- Provide counseling and emotional support to patients and families
- Assist with advance care planning and coordination of community resources
- Participate in interdisciplinary team meetings
- Document services accurately and timely in the EMR HCHB (Homecare Homebase)
- Maintains the agency s mission, philosophy, and core values.
- Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety, and emergency management.
- Always maintains patient confidentiality, including all HIPAA regulations.
- Licensed or registered in the State of Michigan.
- Must pass a criminal background check & MVR check.
- Completed health statements acknowledging the ability to perform the duties of the position.
- Valid state drivers license.
- Proof of current automobile insurance.
- TB testing per agency policy;
- Ability to work independently, make accurate, and at times, quick judgments. Ability to respond appropriately to crises outside of a hospital setting. Acceptance of and adaptability to different social, racial, cultural, and religious modes.
- Minimum 2 years of experience as a social worker, preferred. Active patient contact within the past three years is preferred.
- MSW Degree from a school of social work accredited by the Council on Social Work Education (CSWE), and one year of experience in a health care setting.
- Baccalaureate Degree in social work (BSW) from a school of social work accredited by the CSWE, and one year of experience in a health care setting and be supervised by an MSW from a school of social work accredited by the CSWE and who has one year of experience in a health care setting. If the BSW is employed by The Care Team before December 2, 2008, he/she is exempted from the MSW supervision requirement.
- Have a Baccalaureate Degree in psychology, sociology, or other field related to social work, and at least one year of social work experience in a health care setting and be supervised by an MSW from a school of social work accredited by the CSWE and who has one year of experience in a health care setting.
