Social Worker - Case Management

Overview

JFK Memorial Hospital has provided medical care to residents in the Coachella Valley since 1966. A group of physicians founded the hospital; one of JFK’s cross streets is named in honor of Dr. Reynaldo Carreon. JFK Memorial Hospital is a 145-bed acute-care hospital that offers a variety of services including emergency care, orthopedic and joint replacement, cardiovascular services, maternity and pediatric services, ambulatory surgery, imaging, and outpatient rehabilitation.

• Screenings and assessments of patients working collaboratively with Case Management to facilitate optimal discharge.
• Grief work and supportive therapy with patient and family regarding end-of-life decisions and critical illness.
• Function as a resource and referral service to patients and families.
• Coordinate with outside agencies including but not limited to APC/CPS and psychiatric services.
• Participate in community meetings/associations where social service presence is required.
• Supervise Social Service interns as requested.
• Will be certified by Riverside County as designated 5150 evaluator at JFK Memorial Hospital.

• M.S.W. and current license as Clinical Social Worker in the State of California
• CPR
• Two (2) years experience in an Acute Care Hospital
• Familiarity with psychiatric presentations and mental health services including 5150 criteria and process
• Good communication skills and ability to work with others
• Knowledge of local community resources and procedures for access
• Spanish speaking preferred
