Responsibilities

Assess patient conditions and develop individualized care plans.

Administer medications, monitor symptoms, and provide skilled nursing care.

Perform wound care, IV therapy, and other medical treatments as needed.

Educate patients and families on disease management, medication use, and care techniques.

Monitor and document patient progress, updating care plans as necessary.

Coordinate with physicians, therapists, and other healthcare providers to ensure seamless care.

Provide emotional support and guidance to patients and their loved ones.

Required Qualifications

Current RN license

BLS (other certifications as required by facility)

Two years of recent Home Health RN experience

Strong communication and adaptability skills
