The Intensive Case Manager provides comprehensive case management and housing support services to individuals or families transitioning into stable housing after experiencing homelessness or housing instability. Services are delivered using a Strengths-Based, Harm Reduction approach within a Housing First model. This role supports clients in addressing socioeconomic, psychosocial, and environmental challenges through thorough assessment, goal setting, resource coordination, and ongoing advocacy.
The Intensive Case Manager empowers individuals to build on their strengths, access needed supports, and achieve long-term housing stability and self-sufficiency. This position reports to the Case Management Supervisor.
Please note that this is a grant-funded, at-will position. While this role is intended to be ongoing, its continuation is subject to the periodic renewal of grant resources.
Example Duties and Activities- Provide comprehensive case management and housing support, including assessment of client strengths, needs, and barriers; crisis intervention; advocacy; and connection to community resources such as medical, mental health, and addiction services.
- Conduct thorough client assessments and collaborate with individuals to develop, implement, and modify individualized housing and service plans focused on long-term stability.
- Assist clients with navigating systems of care, addressing housing-related challenges, and resolving conflicts or crises that may impact housing stability.
- Maintain accurate, timely, and confidential documentation of client information, service records, and case notes in accordance with agency policies and applicable standards.
- Collaborate with community providers, multidisciplinary teams, and internal staff to coordinate services and support client progress toward identified goals.
- Participate in case conferences, staff meetings, trainings, and professional development activities.
- Provide peer support, mentorship, and assistance to team members as appropriate.
- Contribute to program evaluation, research initiatives, and service improvement efforts related to policy, advocacy, funding, and program development.
- Observe all policies related to client confidentiality, ethical conduct, and mandated reporting requirements.
- Support intake, outreach, and engagement efforts within the agency and community as needed.
- Provide ongoing support and advocacy with the goal of ensuring housing sustainability and client success.
