Home Health Registered Nurse

Overview

We are seeking a dedicated and compassionate Registered Nurse (Home Health) to join our client’s growing team in Indiana, Pennsylvania. This position offers the rewarding opportunity to deliver skilled, patient-focused care directly in patients’ homes, helping them recover, manage their health, and maintain independence. The ideal candidate will be a clinically strong RN who values flexibility, autonomy, and meaningful one-on-one patient interactions.

• Perform comprehensive nursing assessments and create individualized care plans
• Administer treatments, medications, and other skilled nursing interventions
• Monitor and document patient progress, adjusting care as necessary to achieve optimal outcomes
• Educate patients and families on care procedures, medications, and home safety practices
• Coordinate care with physicians and other members of the interdisciplinary team
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

• Monday through Friday, 8:00am–5:00pm
• On-call rotation every 4–8 weeks (Saturday and Sunday, 8:00am–5:00pm for admissions as needed)
• Two weekdays off following on-call weekends to balance workload
• Flexible scheduling with minimal weekend requirements
• This is a Pay Per Visit position

• Local travel within the Indiana, PA area (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Pennsylvania
• Minimum of 1 year of professional RN experience (home health preferred)
• Current BLS or ACLS certification through the American Heart Association (assistance available if needed)
• Valid driver’s license, reliable vehicle, and current auto insurance

• Prior experience in home health or community-based nursing
• Excellent communication, assessment, and time management skills
