Summary
Opening is located at 2669 Cold Springs Road clinic of Richard L. Roudebush VAMC Indianapolis - IN within the Geriatrics service. The Geriatric Resources for Assessment and Care of Elders (GRACE) Outpatient is seeking an Advanced Practice Nurse (APN) / Nurse Practitioner (NP) to provide full range of services emphasizing quality - timeliness - and advancement of the Veterans Affairs (VA) mission.
The NP provides direct Veteran care in patient homes and coordination of care within the GRACE Program.
- Functions or Scope of Assigned Duties:
Responsible for case management - assessment - triage - Veteran education - resource coordination - medication and treatment coordination - and care planning - Plans and delivers technically competent care in the home of Veterans with complex needs - including health promotion - health prevention - and geriatric care
- Utilizes interventions - which maximize health capabilities and reduces unnecessary urgent care and emergency room visits - per Veteran's goals of care
- Makes home visits to Veterans in the program
- Collaborates in the development - implementation - evaluation - and revision of policies - procedures - and/or guidelines
- Demonstrates performance and leadership that is broad enough to improve the care for the GRACE Program and facility
- Provides peers with formal or informal constructive feedback for improvement
- Supports colleagues and other nurses through geriatric knowledge sharing to provide safe - quality nursing care
- Fosters a safe and supportive environment conducive to the professional development of health care professionals
- Evaluates outpatient outcomes of evidence-based decisions and practice changes for aging individuals - groups - and populations
- Responsible for the documented GRACE outcomes at the program or service level
- May have long periods of continued driving - walking - standing - stooping - sitting - bending - pulling and climbing steps
- An ability to lift objects such as laptop cases - nurse bag and other home care nurse supplies in and out of vehicles and into homes is necessary
- Must be capable of working with a team effectively in stressful situations and demonstrate ability to shift priorities based on Veteran - environmental and program needs
