Schedule

Monday – Friday, 8:00 am - 4:30 pm

No weekends. No Holidays.

Responsibilities Diagnostic & Documentation Support
  • Prepare, complete, and manage Certificates of Diagnosis (COD) for autism, ADHD, and other developmental/behavioral conditions.
  • Gather necessary clinical information, past records, therapy notes, and school documents required for diagnosis documentation.
  • Coordinate signatures, provider review, and distribution of diagnostic paperwork to families, schools, and community programs.
  • Maintain templates and ensure forms meet state and insurance requirements.
Provider Support & Plan of Care Facilitation
  • Assist physicians and APPs with implementing plan-of-care (POC) recommendations, such as:
    • therapy referrals (OT, PT, speech, ABA),
    • follow-up appointments,
    • school-based service recommendations,
    • community resource connections.
  • Draft care plan components or letters for provider review (e.g., medical necessity statements, therapy orders).
  • Track outstanding tasks requested by providers and ensure timely completion.
Care Coordination & Navigation
  • Help families navigate developmental resources including:
    • early intervention,
    • school IEP/504 processes,
    • behavioral therapy options,
    • community developmental disability services,
    • social work referrals.
  • Serve as a point of contact for families needing clarification on next steps in care.
  • Maintain updated resource lists tailored to developmental needs.
Communication & Triage (Non‑Clinical)
  • Manage incoming phone calls and inbox messages related primarily to:
    • follow‑up on diagnostic paperwork,
    • clarification of provider recommendations,
    • coordination of referrals,
    • appointment pathway questions.
  • Route clinically complex questions to appropriate providers or triage staff.
  • Provide families with written or verbal summaries of care plans following visits.
Multidisciplinary Collaboration
  • Work closely with physicians, NPs/PAs, psychologists, social workers, schedulers, and therapy teams to ensure cohesive patient support.
  • Participate in care conferences or team huddles to:
    • identify barriers for families,
    • follow up on outstanding care‑plan items,
    • streamline documentation workflows.
Requirements
  • Requires an Associates of Nursing (ASN);
    Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program. Exceptions:
    Candidates who have not graduated from a nationally accredited program may be considered if they possess more than two years of relevant clinical experience. Such exceptions are subject to review and approval by the Chief Nursing Officer (CNO).
  • Requires Basic Life Support certification through the AHA.
  • Other advanced Life Support certifications may be required per unit/department specialty according to patient care policies.
  • May require ability to assess patients without face‑to‑face interaction.
  • Requires awareness and understanding of legal implications of patient care activities and performance as a Registered Nurse.
