The Field Registered Nurse (RN) provides skilled nursing services to patients in their homes across Indiana, serving as the clinical lead for care delivery. The RN is responsible for patient assessment, care planning, supervision of LPNs and unlicensed staff, and coordination with physicians and interdisciplinary team members. Care is delivered in compliance with the Indiana Nurse Practice Act, physician orders, Medicaid (IHCP), HCBS Waiver, and agency policies.
DescriptionKey Responsibilities:
- Perform comprehensive patient assessments and develop individualized care plans in collaboration with physicians.
- Provide skilled nursing care, including wound care, IV therapy, medication management, and chronic disease monitoring.
- Supervise, delegate, and direct care provided by LPNs, home health aides, and other caregivers in accordance with Indiana regulations.
- Document all patient encounters, interventions, and progress accurately in line with IHCP, CMS, and agency standards.
- Communicate regularly with physicians, case managers, and interdisciplinary team members to coordinate and adjust care plans.
- Conduct patient and family education regarding disease management, medication adherence, and preventive care.
- Ensure compliance with agency policy, state and federal regulations, and home health quality standards.
- Participate in on-call rotation and provide emergency nursing interventions as needed.
Qualifications:
- Current and unrestricted Indiana RN license.
- Minimum of 1 year of clinical nursing experience (home health or community health strongly preferred).
- Strong knowledge of Indiana Medicaid (IHCP) documentation, HCBS waiver requirements, and home health compliance.
- Excellent clinical assessment, critical thinking, and leadership skills.
- Ability to supervise and mentor LPNs and aides.
- Reliable transportation and valid driver’s license required.
