The Field Licensed Practical Nurse (LPN) delivers direct nursing care to patients in their homes across Indiana under the supervision of a Registered Nurse (RN) and in accordance with the Indiana Nurse Practice Act, physician orders, and agency policies. The LPN promotes patient independence, provides skilled care, and ensures compliance with Indiana Medicaid (IHCP) and Home Health regulations.

Key Responsibilities:

• Provide direct skilled nursing care in accordance with the Indiana Nurse Practice Act, including medication administration, wound care, treatments, and monitoring of vital signs.
• Carry out the plan of care developed by the supervising RN and physician.
• Document patient care, interventions, and progress accurately in accordance with IHCP and agency standards.
• Educate patients and families on medication, treatment, and health management within scope of practice.
• Report changes in patient condition promptly to the supervising RN and physician.
• Participate in interdisciplinary team meetings and follow Indiana HCBS and home health regulatory guidelines.
• Adhere to infection control, patient safety, and agency compliance requirements.

Qualifications:

• Current and unrestricted Indiana LPN license.
• At least 1 year of clinical nursing experience (home health or community-based preferred).
• Knowledge of Indiana Medicaid (IHCP) documentation and HCBS waiver compliance preferred.
• Strong clinical judgment, organizational, and communication skills.
• Ability to work independently in field environments throughout Indiana.
• Valid driver's license and reliable transportation required.
