Earn a $1,500 sign‑on bonus along with competitive compensation and flexible scheduling. Someone completing 20–100 visits per month can earn $1,800–$11,500, depending on state assessment rates of $90–$115.
ResponsibilitiesDuring a visit that can last up to one hour, Matrix Providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into overall well‑being.
- Conduct adult/geriatric assessments, including medical history, diagnosis and treatment, health education, physician referrals, case‑management referrals, follow‑up, and clear documentation according to Matrix guidelines and protocols.
- Administer and order point‑of‑care and lab‑based testing as needed.
- Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with primary‑care physicians on patient education and follow‑up as necessary.
- Provide services at the member’s place of residence, which may be their home or a skilled nursing facility.
- Flexibility – Visits are scheduled based on your availability from Sunday through Saturday, 8 am to 7 pm.
- Competitive pay based on the number of assessments you complete. Those completing 20–100 visits per month earn $1,800–$11,500, depending on state rates of $90–$115 per assessment.

- Education and support – comprehensive onboarding, a preceptor, 1:1 feedback, and free continuing‑education access.
- Additional licensure – support in obtaining state licensure and credentialing in neighboring states.
- Full‑time (FT) opportunities – PRN providers can transition to FT roles and receive a FT sign‑on bonus.
- Master’s degree or commensurate experience and satisfactory completion of NP licensure.
- Current NP licensure in state of practice, including prescription authority or ability to obtain it.
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- One year of NP experience preferred; new graduates encouraged to apply.
- Valid state driver’s license for state of residence.
- Automobile insurance coverage required: bodily injury $100,000 per person/$300,000 per incident; property damage $50,000; collision and comprehensive deductibles $1,000 or less.
- Strong computer skills and familiarity with employee health/medical‑record software.
