Job Summary: We are seeking a compassionate and dedicated LPN Private Duty Nurse to provide professional, high‑quality care to our patients in the comfort of their homes. As an LPN, you will be responsible for delivering medical and nursing care, monitoring patient health, and providing emotional support to both the patient and their family members. This role requires someone who is empathetic, highly skilled, and committed to improving the quality of life for patients requiring long‑term care.
Key Responsibilities- Provide direct nursing care to patients in their homes according to their care plan, including but not limited to administering medications, monitoring vital signs, and performing medical procedures as required.
- Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and feeding.
- Develop and implement individualized care plans, in collaboration with physicians, family members, and other healthcare providers to coordinate home health services.
- Perform wound care, catheter care, and other necessary medical tasks.
- Monitor the patient’s condition and report changes to the physician or family as necessary.
- Provide education to patients and their families/caregivers on diagnoses and care plans regarding healthcare needs and home care instructions.
- Ensure patient comfort and safety in the home environment, making necessary adjustments as needed.
- Assist with mobility and transfers, using proper body mechanics and assistive devices.
- Provide emotional support and companionship to the patient and family, helping reduce feelings of isolation.
- Maintain accurate and timely records of patient care, including progress notes and documentation of treatments.
- Ensure adherence to all safety protocols, infection control practices, and relevant healthcare regulations.
- Collaborate with other healthcare professionals, including physical therapists, occupational therapists, and social workers, to ensure comprehensive patient care.
- Licensed Practical Nurse (LPN) with a valid state license.
- Proven experience in home care or long‑term care settings preferred.
- Experience in wound care, medication administration, and performing nursing assessments.
